Job Summary

We are seeking a highly skilled Licensed Practical Nurse to serve as a Nursing Supervisor at Yale New Haven Health. As a key member of our team, you will be responsible for directing and supervising the nursing care and related activities on the unit, ensuring that our patients receive the highest level of care.

Key Responsibilities
  • Leadership and Supervision: Direct and supervise the nursing care and related activities on the unit, ensuring that our patients receive the highest level of care.
  • Policy Implementation: Implement facility policies and procedures, interpreting existing policies and procedures to nursing assistants, residents, families, and physicians.
  • Compliance and Reporting: Assume responsibility for compliance with Federal, State, and local regulations within the assigned unit, obtaining report from the nurse being relieved and implementing appropriate follow-up action as necessary.
  • Resident Care: Conduct resident rounds, direct care for Certified Nursing Assistants (CNAs), and report problems to the nursing supervisor and initiate corrective action.
  • Medication Administration: Administer and document medications and treatments per the physician's order and facility policy.
  • Data Collection and Documentation: Perform data collection as directed by the registered nurse or shift supervisor, documenting medications administered or refused, effectiveness of PRN medications, treatments administered, wound care given, and general nursing notes.
  • Counseling and Evaluation: Counsel nursing assistants and recommend disciplinary action, when necessary, to the Shift Supervisor, participating in the employee evaluation process by providing information on the clinical performance of CNAs.
  • Communication and Teamwork: Demonstrate excellent communication and interpersonal skills, consistently working cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers.
Requirements
  • Education: Graduated from an approved Practical Nurses Education Program.
  • Experience: At least one year of work experience in rehabilitative, psychiatric, or geriatric nursing preferred.
  • Licensure: A Licensed Practical Nurse licensed in the State of Connecticut.
  • Skills: Excellent communication and interpersonal skills, ability to maintain standards of care rendered at Grimes Center in accordance with the State of Connecticut Public Health Code, the governing body of the facility, and the State and Federal agencies.
