Job Summary

DIRECTION HOME AKRON CANTON AREA AG is seeking a skilled Care Management Supervisor to oversee the operations of our Care Management unit. This role is responsible for managing a team of Care Management staff, ensuring the highest quality of life for our consumers, and addressing their health and safety needs.

Key Responsibilities
  • Manage a team of Care Management staff, including orienting and training new staff, communicating with team members, and performing supervisory/quality assurance reviews.
  • Collaborate with medical and behavioral health leadership to ensure effective care coordination and utilization management.
  • Develop, review, and revise clinical processes, policies, and procedures to ensure compliance with program criteria, policies/procedures, and regulations.
  • Manage statistical record keeping and required reporting of Care Management programs.
  • Serve as a liaison between senior leadership and Care Management staff to implement program operations.
Requirements
  • Bachelor's degree or commensurate work experience (4 years); and Licensed Social Worker or Registered Nurse in the state of Ohio.
  • Preferred: One year of supervisory experience, 2 years of experience within the HCBS department, Lean Six Sigma Green belt certified, and CM certification.
