Job Overview

Maximus is seeking a skilled Field Registered Nurse Assessor to provide high-quality home assessment services to vulnerable populations in the Bronx, NY area.

Key Responsibilities
  1. Conduct in-home, in-facility, or telehealth assessments to evaluate individuals' health statuses, strengths, care needs, and preferences using the UAS-NY assessment tool.
  2. Establish and maintain positive relationships with consumers, caregivers, and facility staff.
  3. Review medical documentation, reporting any concerns, conflicting information, or issues that may arise during the evaluation process.
  4. Enter assessment data into the UAS-NY online application and transmit as required.
Requirements
  • An active RN license is required.
  • A minimum of 2 years of clinical experience and 2+ years of experience in-home care are also necessary.
  • The ideal candidate will have prior experience using the UAS-NY Community Assessment Tool.

Salary and Location

This position offers an estimated annual salary range of $107,000 - $118,000 based on location and relevant work experience. The role requires reliable transportation, and Maximus provides company-issued cellphone and computer equipment. A private and secure home office workspace is also necessary.
