Clinical Overview

Matrix Medical Network offers a comprehensive range of clinical services, empowering primary care providers and at-risk health plan members with the tools and knowledge to effectively manage their health at home.

We break through traditional barriers to care by meeting those in need where they are, providing older adults and other vulnerable individuals with improved quality of care, enhanced health outcomes, and the identification of potential chronic conditions that may otherwise go undiagnosed.

About the Role

In this role, you will conduct Adult/Geriatric Assessments, medical history reviews, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.

Responsibilities include:

  • Conducting assessments, reviewing medical histories, diagnosing conditions, treating patients, educating them on health matters, referring them to physicians or case managers, following up on cases, and documenting all interactions in accordance with Matrix protocols.
  • Collaborating with healthcare professionals, including physicians, case managers, social workers, and caregivers, to provide comprehensive care.

This position requires strong communication skills, both written and verbal, to effectively interact with patients, clients, and colleagues. Additionally, you must be comfortable adapting to changing circumstances and possess excellent time management skills.

The successful candidate will have a Master's degree in Nursing, current RN and NP licensure in the state of practice, prescription authority, board certification by the AANP, ANCC, or AACCN, and at least one year of experience as a Nurse Practitioner.

Benefits and Compensation

This part-time position comes with a competitive hourly rate of $90.00-$95.00, depending on the county, and a mileage reimbursement plan. While there are no benefits provided for PRN/Part-Time employees, you will be eligible for our 401(k) program and limited voluntary benefits.
