Job Description

This Patient Advocacy Coordinator position requires a strong understanding of social work principles and practice, as well as excellent communication and organizational skills. The successful candidate will be able to work independently and as part of a team to ensure the best possible outcomes for our patients.

Key Responsibilities:

  • Assess patient needs and develop individualized plans to address those needs.
  • Collaborate with the healthcare team to ensure seamless coordination of care.
  • Communicate effectively with patients, families, and healthcare providers to facilitate smooth transitions and optimize patient outcomes.
  • Stay current with relevant laws, regulations, and best practices in social work and healthcare.

Requirements:

  • Masters degree in Social Work from a school accredited by the Council on Social Work required.
  • One year of post-Masters degree experience in a healthcare setting preferred.
  • Current state licensure from the Texas State Board of Social Worker Examiners is required.

Estimated Salary: $75,000 - $95,000 per year, depending on experience and qualifications.
