We are seeking a skilled Case Manager/Social Worker to join our team

Estimated Salary: $25/hour

Job Description:The Case Manager/Social Worker will perform a variety of assignments in the areas of assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients behavioral health needs. Key responsibilities include:

  1. Establishing and maintaining effective working relationships with programs, community stakeholders, and officials of public and private sector;
  2. Conducting site visits for service providers;
  3. Providing case management services to children, youth, and their families referred through the Department of Juvenile Services (DJS) and referrals from other child-serving entities;Initiating contact with the family within 48 business hours of receipt of the referral to schedule an initial home visit and explain the purpose of the program;Meeting with the parent/guardian and youth within 10 business days;
  4. Completing an intake assessment using the CAFAS as part of the face-to-face meeting;
  5. Utilizing the assessment process to continually measure the youth progress and provide outcome data on the extent to which family functioning improved over the course of service provision;
  6. Providing crisis intervention; addressing parent/child conflict; teaching pro-social skills; improving relationships between the family, community, and school;Providing administrative support to the Children, Youth and Families Division;
  7. Assisting with the development, preparation, and submission of monthly and quarterly reports for funding sources;
  8. Serving as support staff to the Local Management Board (LMB) and standing committee meetings; attending meetings, preparing correspondence, recording minutes, and/or other materials as needed;Performing other tasks and duties as assigned that may not be specifically listed in the class specification or position description but fall within the general occupational category and responsibility level typically associated with the incumbents class of work.

Requirements:To be considered for this role, you must have:
  • A Bachelor's Degree in a related field, such as Human Services, Sociology, or Psychology;A minimum of [insert experience] years of experience working with families, children, infants, toddlers, and youth;
  • Excellent time management and organizational skills; results-oriented; exceptional attention to detail; excellent written and verbal communication skills; good process improvement and mathematical skills; proficient in English; however, a good knowledge of additional languages will be a definite plus;
  • Working knowledge of Microsoft Office; ability to work under pressure;
  • Must be legally authorized to work in the United States of America; sponsorship will not be a part of this contract.
