We are seeking a compassionate and dedicated Registered Nurse (RN) to provide quality home health or hospice care to our community-based patients in Denton, TX and the surrounding areas.
At Jet Health, Inc / Trio Home Health & Hospice, we strive to create a sense of belonging for all our employees and are committed to offering services that ensure our geriatric patients have a safe home life with comfort, independence, and dignity.
Key Responsibilities:
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions, reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
Requirements:
- Current State or Compact Registered Nurse (RN) License in hand
- A minimum of one-year clinical experience is preferred
- OASIS knowledge highly preferred for home health applicants
- Demonstrate knowledge and skillset in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
- HCHB EMR knowledge is desired
