Medicaid Program Service Coordinator

We are seeking a dedicated Case Manager/Service Coordinator to support a dual population program. This role involves managing a mixed caseload, focusing on an aging demographic wishing to remain in their homes, as well as individuals recovering from Traumatic Brain Injury (TBI). The compensation ranges from $20 to $23 per hour, and candidates must possess a Bachelor's degree along with a minimum of three years of relevant experience.
The ideal candidate will manage a full-time caseload of approximately 20 clients.
Key ResponsibilitiesThe Service Coordinator will be tasked with:
- Conducting formal reviews, updates, and timely submissions of Service Plans (ISP, RSP, and addendum) to the appropriate review body;
- Organizing and leading Team Meetings, including 30-day and 6-month reviews;
- Maintaining comprehensive records for a minimum of seven years post-termination of waiver services;
- Ensuring the completion of the UAS annually, meeting the required Level of Care;
- Keeping accurate records of waiver transportation services;
- Providing all waiver service providers and participants with the latest approved Service Plan;
- Staying informed about all approved waiver service providers in the designated region;
- Conducting in-home visits with participants at least quarterly, and virtual visits monthly;
- Preparing and reviewing housing documentation for annual audits related to housing subsidies.
To qualify for the Service Coordinator position, candidates must meet one of the following criteria:
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) as per NYS Education Department regulations;
- Possession of a Doctorate or Master’s degree in Social Work or Psychology;
- Licensed Physical Therapist, Registered Professional Nurse, Certified Teacher of Students with Disabilities, Certified Rehabilitation Counselor, Licensed Speech Pathologist, or Licensed Occupational Therapist, all licensed by the NYS Education Department;
- At least one year of experience in case management/service coordination, information and referral services, or service brokering for individuals with disabilities or TBI, or a Bachelor's degree with three years of similar experience.
