Job Summary

We are seeking a highly skilled Telephonic Nurse Case Manager II to join our team at Elevance Health. As a Telephonic Nurse Case Manager II, you will be responsible for providing comprehensive care management services to members with complex and chronic care needs.

Key Responsibilities
  • Care Management: Assess, develop, implement, coordinate, monitor, and evaluate care plans to optimize member health care across the care continuum.
  • Telephonic Services: Provide telephonic care management services to members, including assessments, care planning, and coordination of care.
  • Collaboration: Work closely with medical directors, physician advisors, and other healthcare professionals to develop and implement care management plans.
  • Communication: Effectively communicate with members, families, and healthcare providers to ensure seamless care coordination.
  • Problem-Solving: Identify and resolve complex care management issues, including claims and service issues.
Requirements
  • Education: Bachelor's degree in a health-related field.
  • Experience: Minimum of 5 years of clinical experience, with a focus on case management or a related field.
  • Licensure: Current, unrestricted RN license in applicable state required.
  • Multi-State Licensure: Multi-state licensure required if providing services in multiple states.
Preferred Qualifications
  • Certification: Certification as a Case Manager (CM) or a related field.
  • Compact RN License: Compact RN license preferred.
  • Acute Care Experience: Minimum 2 years' experience in an acute care setting.
  • Managed Care Experience: Experience in managed care, including care coordination and case management.
