Job Summary

Grace and Glory Hospice is seeking a compassionate and skilled Home Health Aide to provide personalized care to patients in their homes. As a key member of our interdisciplinary team, you will work under the direction of a registered nurse to deliver high-quality care and support to patients and their families.

Responsibilities
  • Personal Care Services: Provide bathing, dressing, toileting, feeding, and other personal care services to patients according to their individualized care plans.
  • Care Documentation: Accurately document all care provided to patients through our online system, HospiceMD.
  • Communication: Report any changes or concerns to the Case Manager or designee, and maintain open communication with patients, families, and healthcare team members.
  • Equipment Use: Utilize durable medical equipment as needed to support patient care.
  • Team Collaboration: Participate in team and staff conferences to foster a cooperative effort among personnel and ensure seamless patient care.
  • Respite Care: Provide respite care to patients' families and caregivers as needed.
  • Quality Assurance: Participate in Quality Assurance and Performance Improvement (QAPI) activities to ensure high-quality patient care.
Requirements
  • Home Health Aide Certification
  • CPR Certification
  • Physical Form: Signed by a doctor (provided by Grace and Glory Hospice)
  • Recent TB Test and Immunizations or Titers
  • Two Forms of ID
  • Auto Insurance
  • Reliable Transportation
Working Conditions
  • The ability to access patients' homes, which may not be routinely wheelchair accessible, is required.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
  • Physical activities will include walking, sitting, stooping, standing, and minimal to maximum lifting of patients and turning of patients.
