Job Summary

We are seeking a Certified Home Health Aide (CHHA) to join our team. As a CHHA, you will provide occupational therapy assessment, services, direction, and care to patients in your region. You will evaluate the patient's home environment and functional status, and participate in the development of the total plan of care.

Responsibilities
  • Provide occupational therapy assessment, services, direction, and care to patients in your region
  • Evaluate the patient's home environment and functional status
  • Participate in the development of the total plan of care
  • Work collaboratively with other healthcare professionals to ensure high-quality patient care
Qualifications
  • Active CA Certified Home Health Aide (CHHA) license
  • A minimum of one year of patient care experience; preferable in the hospice setting
