Nurse Care Manager 2

Job Summary:
The Registered Nurse (RN) Care Manager provides a holistic, patient-centered approach in caring for patients seen through Project HOME's Healthcare Services. Key responsibilities include:
- Collaborating with participants, Project HOME stakeholders, and primary care providers to provide coordinated care.
- Assessing, triaging, and responding to patients' acute, chronic, and preventive health needs.
- Providing mentorship and guidance to new nurse care managers.
Care Management:
The RN Care Manager coordinates care with primary care providers, behavioral health providers, and other providers to implement individual care plans. This includes:
- Information-sharing, goal setting, and pre-visit planning with participants.
- Exploring gaps in care and working to eliminate them.
- Providing education on health-related topics, such as harm reduction, disease-specific management, and healthier lifestyle choices.
Direct Clinical Care:
The RN Care Manager demonstrates clinical excellence, flexibility, and critical thinking in providing direct care to patients. This includes:
- Performing clinical assessments and procedures using universal precautions.
- Providing staff training and education on medication monitoring and other critical topics.
- Facilitating vision screenings and assisting patients with obtaining corrective eyewear.
Documentation:
The RN Care Manager ensures accurate and timely documentation of care management data in the Electronic Health Record (EHR). This includes:
- Obtaining signed written authorizations from participants to coordinate and share information with other healthcare providers.
- Maintaining a training, certification, and continuing education activity log.
Requirements:
The ideal candidate will have:
- A Bachelor of Science in Nursing (BSN) degree.
- At least 3 years of experience as a nurse care manager.
- An active RN license in Pennsylvania.
- BLS certification.
