Position Overview:


The Eligibility Registered Nurse plays a crucial role within the In-Home Services division, collaborating closely with the local Enrollment team to assess whether potential participants fulfill the criteria for enrollment in the PACE program.


This role involves scheduling and conducting home evaluations to accurately assess the level of care required, thereby determining participant eligibility.


Key Responsibilities:


- Conduct home visits and perform level of care assessments.
- Complete necessary documentation in partnership with the InnovAge Primary Care Physician, ensuring all supporting materials for enrollment are compiled effectively.
- Engage in outreach initiatives to promote program awareness.


- Act as a vital member of the Interdisciplinary Team (IDT), coordinating appointments for individuals needing further evaluations by other IDT members.


- Participate in case management and peer review of client records, providing insights and consultation to case managers and other team members on medical and nursing matters.


- Maintain a comprehensive understanding of eligibility, enrollment, and program specifications related to Federal and State health coverage programs, particularly Medicare and Medicaid, as they pertain to the Program of All-Inclusive Care for the Elderly (PACE).


- Support the In-Home Services team by assisting with in-home assessments, reassessments, supervisory visits, and routine RN care as required.


Qualifications:
Required:
- Associate degree from an accredited nursing institution.
- Valid Registered Nurse License in the state of employment.
- Current CPR certification or willingness to obtain certification.
- Minimum of 3 years of healthcare experience, with a focus on geriatrics.
- At least 1 year of experience working with elderly or frail populations.
- Reliable personal transportation, a valid state-issued driver's license, a clean driving record, and auto insurance as mandated by law.

Preferred:
- Bachelor’s degree in nursing.
- Certification as a Gerontological Nurse.
- Previous experience with State and Federal regulatory bodies.
- Bilingual capabilities.
