Hospice Home Care Nurse Specialist

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Home Care team at Illuminus. As a Hospice Home Care Nurse Specialist, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.
Key Responsibilities
Provide skilled nursing care to patients with complex medical needs
Develop and implement individualized care plans to meet the unique needs of each patient
Collaborate with the interdisciplinary team to ensure comprehensive care and support
Educate patients and their families on disease management and self-care techniques
Requirements
Current RN licensure in the State of Wisconsin
Associate's degree in Nursing (or higher)
Experience working in hospice or palliative care (preferred)
