Community Support Coordinator

The Community Support Coordination Services are founded on ethical standards that honor the autonomy, dignity, privacy, and rights of individuals. Each Case Coordinator at Chapman Partnership delivers essential support services to families and individuals, addressing their social and emotional requirements as they relate to their medical, psychological, economic, and spiritual well-being, while facilitating safe transitions between different living environments.
KEY RESPONSIBILITIES:
- Assist families residing in temporary accommodations by connecting them with suitable services and developing personalized service and goal plans.
- Conduct assessments within 72 hours of a family's admission into temporary housing (excluding weekends).
- Engage in case discussions regarding the resident's status and discharge planning.
- Provide community-based services aimed at restoring families to a stable social environment, leveraging community resources to help residents achieve stability and independence.
- Maintain up-to-date case notes and goal plans for each assigned family, documenting client activities regularly.
- Offer employment and housing assistance, ensuring follow-up services or facilitating transfers to shelters as needed.
- Support families in developing housing plans and refer them to follow-up case managers for ongoing planning and referrals.
- Prepare and maintain comprehensive case records for each family; compile and submit statistical reports and participate in customer satisfaction assessments; analyze productivity and performance metrics.
- Perform additional tasks or assignments as required.
- Bachelor's degree in social services, human services, or a related field from an accredited institution, or equivalent experience.
- A minimum of one (1) year of practical experience as a case manager in a social services setting, or equivalent experience, with four (4) years of experience substituting for the educational requirement.
- Possession of a valid Florida driver’s license and a clean driving record.
- Understanding of social services, counseling, and behavioral sciences.
- Comprehensive awareness of the challenges faced by our residents.
- Maintaining a satisfactory driving record.
- Experience working with vulnerable populations.
- Ability to evaluate situations, consider alternatives, and determine appropriate actions.
- Capability to build and sustain effective working relationships with public and private organizations, clients, and the community.
- Cultural sensitivity towards diverse client groups.
- Professionalism, flexibility, respectfulness, and composure.
- Strong teamwork skills.
- Ability to lead by example.
- Familiarity with referral procedures to appropriate services provided by other organizations.
- Exceptional verbal and written communication abilities.
- Proficiency in computer usage.
- Solid management skills in social services.
- Bilingual in English and Spanish and/or English and Creole is preferred.
