Physical Therapist

Adoration Home Health and Hospice is seeking a skilled Physical Therapist to join our team in Lafayette, TN. As a Physical Therapist, you will make intermittent home-visits to our skilled nursing patients, providing high-quality care and support. About the Role:
As a Physical Therapist with Adoration Home Health and Hospice, you will be responsible for performing physical therapy evaluations and treatments, developing treatment plans, and maintaining communication with the treatment team, physician, patient, and family members. You will also supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities. If you have a minimum of one year of PT experience and a current, unrestricted license as a Physical Therapist in the state of operation, we would like to hear from you.

Responsibilities:
- Perform physical therapy evaluations and treatments based on physician orders, including recommendation and procurement of adaptive equipment
- Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
- Maintain communication among the treatment team, physician, patient, and family members as applicable
- Maintain thorough and timely documentation of services provided to ensure proper billing
- Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act
Qualifications:
- Minimum Bachelor's Degree in Physical Therapy from a college or university with an accredited Physical Therapy program
- Must have a current, unrestricted license as a Physical Therapist in the state of operation
- Must have current BLS or CPR in accordance with state regulations
- Minimum one (1) year of PT experience preferred
