Key responsibilities include:
- Providing case management to a group of patients, conducting assessments, and providing education on wound care management.
- Collaborating with healthcare providers, wound care center staff, and other stakeholders to ensure seamless patient care.
- Managing multiple patients simultaneously, prioritizing tasks, and maintaining a high level of energy and focus.
Requirements include:
- Active Registered Nurse license and state certification.
- Wound care certification desirable, but not required.
- Excellent written and verbal communication skills.
- Prior experience with Microsoft products and EHRs.
