Job Summary

We are seeking an experienced Nephrology Care Coordinator to join our team in providing excellent patient care and support. As a Nephrology Care Coordinator, you will be responsible for coordinating patient care activities, communicating with patients and families, and collaborating with multidisciplinary teams to ensure seamless care transitions.

The ideal candidate will have at least 9 months of experience as a Registered Nurse (RN) and 6 months of experience in dialysis. You will also possess strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Coordinate patient care activities
  • Communicate with patients and families
  • Collaborate with multidisciplinary teams to ensure seamless care transitions
  • Develop and implement individualized care plans
  • Monitor patient progress and adjust care plans as needed
Salary and Benefits

The estimated salary for this position is $70,000 - $100,000 per year, depending on experience. We also offer a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.

Requirements
  • Bachelor's degree in Nursing (BSN)
  • Current RN licensure in the state where you reside
  • At least 9 months of experience as a Registered Nurse (RN)
  • 6 months of experience in dialysis
  • Strong communication and interpersonal skills
