We are seeking a skilled Social Worker MSW to join our team at Atrium Health. Key Responsibilities- Conduct psychosocial assessments for patients and their support systems to identify needs and develop effective care plans.
- Assist patients in coping with stress related to hospitalization, disability, chronic, or terminal illness, providing emotional support and guidance.
- Collaborate with the Clinical Care Management team to assess and evaluate patients for the appropriate level of care or environment setting to meet their care needs.
- Serve as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of patients' illnesses and their effects on their social support systems.
- Provide information, education, and referrals to patients on community resources and options for post-hospital care, taking into account the patient's age and needs.
- Report suspected cases of child and adult abuse, neglect, or exploitation, serving as a liaison between the hospital, patients, and Department of Social Services during evaluation and investigation.
- Respond to requests for consultative services to patients after normal working hours.
- Masters in Social Work (MSW) required; Licensed Clinical Social Worker (LCSW) preferred.
- NC Certification preferred.
- At least 1 year of professional experience in a hospital or health-related setting.
- Expertise with Data Management Tools.
