Job Opportunity at Custom Home Health Inc

We are seeking a skilled Licensed Practical Nurse (LPN) to join our hospice care team at Custom Home Health Inc. As a LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Responsibilities:

  • Provide accurate and timely documentation of patient care and progress.
  • Assess and educate patients and their families on self-care techniques and information related to their diagnosis and care plan.
  • Utilize equipment and supplies effectively and efficiently, adhering to aseptic technique.
  • Participate in personal and professional growth and development, including orientation, in-service programs, and staff meetings.
  • Collaborate with other disciplines to coordinate hospice services and identify patient needs.

Requirements:

  • Graduate of an accredited practical nurse program.
  • At least one year of experience in hospice nursing, with community/home health or medical/surgical experience preferred.
  • Current licensure as a licensed practical nurse in the state.
  • Valid CPR certification.
  • Licensed driver with a reliable vehicle and insurance.
