We are seeking a skilled Advanced Practice Clinician to join our team in providing high-quality patient care in the comfort of their homes.

About the Role:

  • Conduct in-home assessments, including medical history review, medication reconciliation, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as needed.
  • Identify diagnoses and develop care management plans to improve patient outcomes.
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination.
  • Educate patients on disease management, medication adherence, and healthy lifestyle choices.

Requirements:

  • Active NP or PA license, with ability to obtain by start date.
  • Current national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care.
  • Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions.
  • Reliable transportation for home visits.

Preferred Qualifications:

  • 1+ year of clinical experience in a similar setting.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home health care or home visit experience.
