Key Responsibilities:

Summary

The Lead Case Manager operates under general supervision, guiding the case management team by providing training, support, and structure for participants. This role involves developing program plans, managing participant caseloads, and ensuring adherence to policies and procedures as outlined by the relevant agency and Program Manager.


Primary Duties:
  • Oversee the activities of the case management team, assisting with complex inquiries and providing training to new staff.
  • Manage a caseload of participants, conducting assessments and creating individualized Behavior Change Plans.
  • Prepare tailored programs for participants, establishing measurable goals and reviewing progress regularly.
  • Orient new participants to the program, ensuring they understand the necessary paperwork and procedures.
  • Maintain secure and confidential participant files to facilitate effective case analysis and programming.
  • Engage in staff meetings to share insights and collaborate on solutions to enhance team performance.
  • Uphold confidentiality standards regarding sensitive information related to participants and staff.
  • Report any program violations to the appropriate authorities, ensuring compliance with established protocols.
  • Conduct orientation sessions for new participants, clearly explaining program rules and responsibilities.
  • Adhere to company policies and safety regulations, ensuring a secure environment for all.
  • Communicate effectively with all staff levels, fostering a cooperative team atmosphere.
  • Perform additional duties as assigned.

Qualifications:

Minimum Requirements:

  • High School Diploma or equivalent; a Bachelor's degree in a Human Services discipline is preferred.
  • At least one year of experience as a Case Manager or three years in a related field.
  • Certification in substance abuse or domestic violence is preferred, or the ability to obtain certification.
  • Bilingual in English and Spanish is preferred but not mandatory.
  • Ability to interact professionally in both individual and group settings.
  • Proficient in data collection and analysis, with the capability to generate related reports.
  • Experience in leading a team and implementing collaborative strategies.
  • Strong organizational skills and attention to detail.
  • Basic knowledge of Microsoft Office Suite, including Word, Excel, and Access.
