Your Role:
The Service Coordination Specialist plays a crucial role in leading discussions and documenting the identified requirements and strategies to address those needs. This position involves preparing and submitting necessary documentation to the Department of Aging and Disability Services to help individuals access and sustain essential services and support. Additionally, the Specialist is responsible for overseeing service provision, updating documentation as individual circumstances evolve, and assisting clients in maintaining their benefits.
Your Responsibilities Include:
- Leading and documenting meetings to assess individual needs and formulate actionable steps.
- Conducting quarterly evaluations of service delivery.
- Preparing and submitting required documentation to the Department of Aging and Disability Services to ensure continued access to services.
- Assisting clients in preserving their benefits.
- Ensuring that adaptive aids are procured in compliance with HCS guidelines.
- Notifying the Service Coordinator of any changes in individual needs and service delivery.
- Collaborating effectively with all team members.
- Maintaining open lines of communication with clients, their families, and all team members.
- Participating in a monthly on-call rotation for residential services.
- Performing additional duties as assigned by the Area Director.
Qualifications Required:
- A high school diploma, GED, or equivalent competency assessment along with three letters of recommendation.
- Experience in providing services within the HCS program (PREFERRED).
- Certification in CPR and Prevention.
- Ability to successfully complete Prevention and Management of Aggressive Behavior (PMAB) training.
**To ensure the safety of clients and staff, you must demonstrate the physical skills necessary for effective PMAB. This training can be both mentally and physically demanding. You will learn the principles and methods of PMAB, effective decision-making, communication skills, and, when necessary, the ability to physically intervene in real-life situations involving aggressive behavior. To demonstrate the physical procedures up to intermediate protection, you must be capable of: moving backward, pivoting, pulling with either arm, grasping a wrist, bending forward, and quickly returning to a standing position. Additionally, to perform physical restraint procedures, you must be able to: lift and/or support at least 55 lbs, and while supporting a person, step back, kneel, and gently lower them to the floor; lie in a prone position, and kneel on either knee. ** - A valid Texas State Driver's License.
- Current automobile insurance and the ability to travel to clients as needed.
