Moments Hospice is seeking a compassionate Licensed Social Worker (LSW) to join our Social Service team. As a Hospice Social Worker, you will play a vital role in providing psychosocial support to terminally ill patients and their families.
Key Responsibilities- Provide a wide range of psychosocial services to patients and their families, including counseling, problem-solving, and community referrals.
- Assess patients' and families' psychosocial needs and develop plans to meet those needs.
- Collaborate with the interdisciplinary team to ensure comprehensive care.
- Document patient progress and maintain accurate records.
- Participate in weekly team meetings to discuss patient care and share best practices.
- Bachelor's degree from a school of social work accredited by the Council on Social Work Education.
- Minimum of two years of social work experience in a healthcare setting or social services agency.
- Current Social Worker license.
- Hospice experience is a plus.
- Strong organizational and communication skills.
