We are seeking a skilled and compassionate RN to join our Hospice Home Care team at EvergreenHealth. As a Hospice Nurse, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.
Key Responsibilities- Provide direct patient care and assess the comprehensive nursing needs of assigned patients
- Coordinate patient and family education, including symptom management and end-of-life care
- Initiate patient start of care through established admission process
- Continuously plan, implement, and evaluate patient care, incorporating patient and family into the process
- Participate in interdisciplinary team meetings to develop and implement patient plans of care
- Effectively communicate patient information and changes in condition to team members and physicians
- Bachelor's in Nursing (BSN) required within 5 years of employment
- Licensed as a Registered Nurse in the State of Washington
- Current Healthcare Provider BLS certification
- Current Washington State Driver's license and proof of insurability
