Job Summary
We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team. As an RN Case Manager, you will be responsible for coordinating patient care plans, conducting home visits, and communicating with our interdisciplinary team to ensure seamless care delivery.
Responsibilities
- Perform admission, discharge, and routine home health visits to assess patient needs and develop personalized care plans.
- Coordinate and communicate patient care plans with our interdisciplinary team, including physicians, therapists, and other healthcare professionals.
- Document all phone calls and visits using our electronic medical record (EMR) system.
Requirements
- Current RN licensure in the state of practice.
- Minimum 2 years of experience in home health care or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
