Nurse Practitioner Home Hospice PRN

The Registered Nurse (RN) is a skilled healthcare professional responsible for delivering high-quality patient care in a home health and hospice setting. As a member of our team, you will utilize your expertise to assess, plan, implement, and evaluate patient care, ensuring the best possible outcomes for our patients.
Unit Information:Our Home Health and Hospice team is dedicated to providing compassionate and personalized care to patients in the comfort of their own homes. As a Registered Nurse on our team, you will have the opportunity to develop your clinical skills and make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:- Assess patient needs and develop individualized care plans
- Provide high-quality patient care, including wound care, injection and medication administration, and IV monitoring
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Provide patient and caregiver education to promote health and wellness
- Current RN license in the state of practice
- Basic Life Support (BLS) Certification for healthcare providers
- Current driver's license, insured and reliable transportation, and an acceptable driving record
- Ability to communicate effectively both verbally and in writing
- Bachelor's Degree in Nursing (BSN)
- Previous experience in an Acute Care, Home Health, or Hospice setting
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy.
Expected to lift and utilize full range of movement to transfer patients.
Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment.
May be expected to stand in a stationary position for an extended period of time.
