Job Summary

The Denver Hospice is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Hospice Care Specialist. This is an exciting opportunity to make a meaningful difference in the lives of patients and their families.

Key Responsibilities
  • Provide patient-centered care in accordance with applicable scope and standards of practice, adhering to the policies, values, and mission of The Denver Hospice.
  • Participate in Interdisciplinary Team/Group (IDT/IDG) meetings to ensure seamless coordination of care.
  • Collaborate with RN Case Manager, Charge RN, or assigned RN to ensure Plan of Care (POC) is being followed.
  • Attend IDG/IDT Meetings as assigned and provide continuous monitoring of patient's physical, emotional, and spiritual care needs.
  • Communicate patient and family information, status, and level of care to RN, manager, provider, and other IDT members.
  • Assist with coordination and successful completion of patient transfers.
  • Educate patients, families, caregivers, and community regarding hospice philosophy, palliative, and home health treatments.
Requirements
  • Graduate of an accredited school of practical nursing.
  • One year of experience in hospice, palliative care, and/or home health, medical/surgical, public health, oncology, acute care, or home health.
  • Current Colorado LPN license.
  • Current CPR certification.
  • Valid Colorado driver's license and proof of automobile insurance in accordance with state statute.
Physical Requirements

Ability to lift/push/pull minimum of 35 lbs.
