Position Overview: The Youth Support Services Coordinator plays a pivotal role in overseeing a team of Community Support Specialists and Family Support Providers dedicated to delivering behavioral health support services to children facing serious emotional challenges and their families.
Key Responsibilities:
- Conduct and review assessments to identify the unique strengths and needs of children and their families.
- Develop and review individualized wellness plans tailored to each child's requirements.
- Orient new team members to agency policies and Children's CPRC programs, ensuring they meet competency expectations.
- Facilitate participation in interagency and interdisciplinary teams, fostering strong relationships and effective time management.
- Ensure Community Support Specialists assist families in accessing culturally appropriate services and resources.
- Collaborate with other service providers, educational institutions, and community organizations to maintain open communication regarding the child and family.
- Monitor the delivery of service plans, reassessing the child's status and progress towards their goals.
- Develop and implement crisis intervention and safety plans as necessary.
- Maintain thorough documentation of all aspects of children's services, including healthcare and wellness benefits.
- Provide direct clinical supervision of children's services, including regular individual supervision and team meetings.
- Assist clients with transportation needs as required; driving is an essential function of this role.
- Perform additional duties as assigned, including participation in emergency response and crisis intervention efforts.
Qualifications:
- Bachelor's degree required; Master's degree in a human services-related field preferred.
- Must be a Qualified Mental Health Professional (QMHP) or a Qualified Addiction Professional (QAP) as defined in relevant regulations.
- If not a QMHP, a Bachelor's degree with three years of experience in community support services for individuals with mental health or substance use disorders is required.
- A valid driver's license and agency-specified minimum automobile coverage are necessary.
