Role Overview:
The Advanced Practice Nurse (APN) plays a vital role in our collaborative care environment. Our focus is on delivering high-quality, value-based care that enhances the health outcomes of our patients.

Key Responsibilities:
- Conduct thorough evaluations and provide treatment for patients in line with established care standards.
- Monitor patient care quality and outcomes through data analysis and chart reviews.
- Facilitate coordination of patient services, including specialty referrals and home health care.
- Engage actively with the care team during daily meetings to foster a cooperative practice environment.
- Maintain accurate and timely medical records, collaborating with coding specialists to ensure precise documentation.
- Uphold compliance with all relevant licensing and accreditation standards.

Qualifications:
- A minimum of two to five years of relevant experience is preferred.
- Successful completion of an accredited Nurse Practitioner program with Board Certification.
- An active, unrestricted Nurse Practitioner license in the state of practice.
- Excellent communication skills, both written and verbal, with a strong emphasis on teamwork and integrated care.

Preferred Qualifications:
- Active DEA license and familiarity with Medicare guidelines.
- Experience managing a Medicare Advantage patient panel and understanding of coordinated care best practices.
