We are seeking a skilled and compassionate Registered Nurse to join our team at the Texas Department of Aging & Disability Services. As a Direct Care Nurse, you will play a vital role in providing high-quality patient care and support to individuals with intellectual and developmental disabilities.
Key Responsibilities- Provide routine level nursing care to individuals residing in a State Supported Living Center setting, including assessment, care, and treatment.
- Evaluate and document healthcare needs and/or issues, and report to the appropriate person(s).
- Communicate effectively with direct care staff, physicians, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team.
- Notify the supervisor and PCP of trends and concerns to ensure consistent, high-quality care for all individuals served.
- Complete data entry, required documentation, and reports, and maintain accurate records.
- Assist with services provided by LVNs and Certified Medication Aides (CMA), as needed, and make recommendations for improvements.
- Participate in facility quality assurance/quality improvement (QA/QI) initiatives, as required.
- Respond to emergency situations to ensure safety and care of individuals and staff.
- Knowledge of nursing techniques and procedures, health care laws and regulations, medical diagnoses and procedures, accepted medical treatment programs, and community health and nursing care principles, practices, and procedures.
- Knowledge of state and federal laws and Intermediate Care Facility for individuals with intellectual disabilities (ICF-IID) rules, regulations, policy, and procedures related to State Supported Living Center or ability to quickly gain that knowledge.
- Knowledge of nursing care standards, the Settlement Agreement, and the Healthcare Guidelines or ability to quickly gain the knowledge.
- Skills in the care and treatment of patients, use of computer/software, and an electronic health record system.
- Excellent verbal and written communication skills and the ability to present ideas clearly and concisely in both written and oral communications.
- Ability to provide patient care, prepare and maintain records, interpret policies and procedures, interpret health care regulations, and recognize patterns of medical necessity treatment.
- Ability to demonstrate a working knowledge of emergency procedures and emergency equipment.
- Ability to appropriately interact with individuals with intellectual/developmental disabilities, with their family members, and with staff members.
- Ability to work independently and productively with all levels of staff.
- Ability to provide support and coaching to LVNs, CMAs, and DSPs.
- Ability to make decisions and recommendations to SSLC leadership, including on-call administrators.
This position requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The employee must be able to work effectively in a team setting, communicate clearly and concisely, and maintain accurate records.
The employee must also be able to lift, push, and pull equipment and supplies as needed, and be able to stand for long periods of time. The employee must be able to work in a variety of settings, including but not limited to, patient rooms, treatment areas, and administrative offices.
The employee must also be able to work a variety of shifts, including but not limited to, day, evening, and night shifts, as well as weekends and holidays.
