Job Title: Licensed Practical Nurse - Harbor Point

Stamford Health, a leading healthcare system in Connecticut, is seeking a skilled Licensed Practical Nurse (LPN) to join our team at the Harbor Point Family Medicine location in Stamford.

Job Summary

The LPN will coordinate patient care, provide mentorship to non-nursing personnel, and serve as a resource to staff and management. The ideal candidate will have excellent communication skills, knowledge of medical terminology, and a minimum of 2 years of experience in a similar position.

Responsibilities
  • Organize patient schedules, greet patients, and document patient concerns and questions.
  • Obtain and accurately document patient vital signs, weight, and report any abnormalities to physicians.
  • Assist with specimen collection and testing as necessary, and document pertinent information.
  • Provide minor treatment as directed by physicians, and assist with minor procedures.
  • Provide education to patients and families as indicated, and document all patient-related activities.
  • Schedule patient testing and pre-certify if necessary, and respond to telephone messages by the end of the shift.
  • End-of-month checks for expired supplies and medication, and stock rooms/maintain inventory.
  • Order supplies as needed, and clean exam/procedure rooms and equipment after each patient.
  • Prepare exam/procedure rooms for the next patient and physician needs, and notify patients of test results as per physician instruction.
  • Attend scheduled staff meetings, in-service education opportunities, and participate in continuing education programs.
Qualifications
  • Graduate of an accredited school of nursing.
  • Valid license as a Licensed Practical Nurse in Connecticut.
  • Maintains a minimum of 6 contact hours annually.
  • Excellent customer service and communication skills.
  • Knowledge of medical terminology.
  • Minimum of 2 years of previous experience in a similar position.
