Job Summary:
We are seeking a highly skilled and dedicated Physical Therapist to join our team at Summit Care. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients, caregivers, and facilities in accordance with the principles and practices of physical therapy guidelines.
Key Responsibilities:
- Evaluate patients to determine their current physical status and develop treatment plans that focus on functional and measurable outcomes.
- Provide physical therapy screening and evaluation to determine patients' current physical status.
- Develop and implement treatment plans that adhere to professional standards of practice.
- Oversee and participate in the maintenance of therapy records.
- Ensure availability of supplies and equipment.
- Maintain appropriate patient schedule, as per established guidelines.
- Provide documentation to maintain quality of medical records, as it relates to reimbursement guidelines, accuracy of billing information, and facility requirements.
- Participate in patient, family, and facility meetings, as appropriate.
- Follow administrative policies and procedures, including those related to emergency procedures, infectious disease control, and safety.
- Coordinate and/or participate in training programs for facility staff, residents, families, and rehabilitation staff.
- Initiate new program development with a focus on functional outcomes to improve the quality of life of facility residents.
- Participate in continuous quality improvement.
Requirements:
- Successful completion of an accredited physical therapy program.
- Valid physical therapy license to practice in the appropriate state(s).
- Effective written and verbal communication skills.
- Effective leadership and customer relations skills.
- Ability to exercise mature judgment with initiative in analyzing and solving problems.

- While performing the duties of this job, the employee shares work space with other employees; therefore, the environment can become crowded and noisy.
