We are seeking a skilled Licensed Practical Nurse to join our team at Professional Case Management. As a key member of our care team, you will provide high-quality nursing care to clients in their homes, working closely with our regional directors, case managers, and other healthcare professionals.
Key Responsibilities:- Deliver nursing care to clients in accordance with federal, state, and local laws, as well as the state-specific scope of practice for licensed practical/vocational nurses.
- Assess client needs and develop individualized plans of care in collaboration with our clinical team.
- Maintain accurate and detailed records of client care, including progress notes and treatment plans.
- Communicate effectively with clients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate of a state-approved school of professional nursing.
- Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
- Ability to work independently and as part of a team.
