Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Professional Case Management. As a key member of our care team, you will provide high-quality nursing care to clients in their homes, working closely with our regional directors, case managers, and other healthcare professionals.

Key Responsibilities:
  • Deliver nursing care to clients in accordance with federal, state, and local laws, as well as the state-specific scope of practice for licensed practical/vocational nurses.

Requirements:
