About the Role:

At Montgomery, we're seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in creating extraordinary client and family experiences, managing best-in-home care plans, and ensuring safe, appropriate, and meaningful care is provided.

Key Responsibilities:

  • Develop and maintain client care plans, creating personalized experiences and communicating effectively with clients, families, care partners, and referral sources.
  • Interface with clients and families to ensure high-quality care is being delivered.
  • Serve as a mentor to caregivers, supporting their care delivery and providing guidance.
  • Maintain a calm, positive demeanor in a fast-paced office environment, effectively multitasking and prioritizing tasks.

Requirements:

We're looking for a highly motivated individual with prior experience in the home care industry or medical office setting. The ideal candidate will possess the following qualifications:

  • High school graduate or equivalent with two years of business experience.
  • Able to work independently, demonstrating sound judgment and decision-making skills.
  • Available for on-call duty outside of normal office hours.
  • Valid driver's license and use of insured automobile.
