Job Summary

The Licensed Practical Nurse Team Leader plays a vital role in delivering high-quality patient care in our skilled nursing facility. As a key member of our team, you will be responsible for coordinating and supporting the activities of certified nurse assistants, while actively participating in the direct care of residents.

Responsibilities
  • Administer nursing care to designated residents under the direction of a registered nurse.
  • Coordinate and support the activities of certified nurse assistants.
  • Participate in the direct care of residents, following specific approaches as stated in the plan of care.
  • Ensure that residents receive comprehensive and compassionate care.
  • Collaborate with the healthcare team to achieve optimal patient outcomes.
Requirements
  • High School diploma or GED.
  • Graduate from an accredited school of practical nursing.
  • Currently registered in New York State as a Licensed Practical Nurse.
  • BLS validation and re-validation in accordance with AHA standards.
  • Long-term care experience preferred.
