- Decentralized Pharmacist Role: Patient Teaching and Communication
- Participate in multidisciplinary rounds to facilitate patient discharge or readmission prevention
- Counsel patients about their drug therapy to ensure safe medication use
- Maintains Professional Competency
- Stay current with trends in pharmacy practice and maintain knowledge of new disease and drug therapies
- Represent the department and profession of pharmacy at committee or task force meetings
- Education
- Bachelor of Science degree in Pharmacy from an approved college of Pharmacy
- Experience
- Zero to six months of experience as a licensed pharmacist
- Licensure
- Registered as a licensed pharmacist or eligible for reciprocity in the State of Connecticut
This position requires manual skills, motor coordination, and finger dexterity. There is some occupational risk and physical effort involved, with the ability to exert up to 35 pounds of force.
