Job Summary:

In this role, you will conduct in-home assessments, perform evidence-based practice screenings, identify diagnoses, formulate medical lists, communicate findings, and educate members on disease processes and medication.

You'll have the ability to achieve work-life balance with flexible scheduling, no on-call, weekends, or holidays required.

Key Responsibilities:
  • Conduct thorough in-home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • Perform point-of-care testing as necessary
  • Identify diagnoses to inform care management and active medical management
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment
  • Communicate assessment findings to inform member's PCP of potential gaps in care
  • Educate members on topics such as disease process, medication, and compliance

Estimated Annual Salary: $143,600 - $169,600
