We are seeking a skilled Occupational Therapist to join our team at Adoration Home Health and Hospice. As a Home Health Occupational Therapist, you will play a vital role in helping our patients recover and regain their independence in the comfort of their own homes.
Key Responsibilities- Conduct thorough evaluations of patients' daily living and work-related skills to identify areas for improvement and develop personalized treatment plans.
- Design and implement community reintegration activities to assist patients in physical and cognitive reconditioning, working closely with the interdisciplinary team to coordinate care.
- Recommend and design adaptive equipment to enhance patients' function in their home, community, and work environments.
- Develop and implement retraining programs to teach patients and their caregivers essential skills, such as dressing, feeding, grooming, and hygiene.
- Provide training and education to patients, caregivers, and healthcare professionals to promote optimal outcomes and independence.
- Collaborate with the rehabilitation team to ensure seamless care coordination and effective problem-solving.
- Document patient progress and outcomes, reporting changes and updates to the clinical team and other stakeholders as needed.
- Bachelor's degree in Occupational Therapy.
- Current professional license and/or certification in Occupational Therapy.
- 1 year of experience in home health or a related field (preferred).
- Knowledge of rehabilitation techniques for complex neurological injuries.
- Effective communication and interpersonal skills.
- Current CPR certification.
- Ability to work independently with minimal supervision and maintain a high level of organization and productivity.
- Reliable transportation and a valid driver's license.
- Physical ability to perform job-related duties, including lifting, standing, bending, and walking.
