We are seeking a compassionate and skilled Private Duty Nursing Professional to join our team at Phoenix Home Care & Hospice. As a key member of our care team, you will provide high-quality nursing care to our clients in the comfort of their own homes.
Responsibilities- Develop, review, and revise individualized plans of care in collaboration with clients, families, and healthcare professionals.
- Provide nursing care in accordance with physician orders, client needs, and payer source requirements.
- Observe, record, and report client reactions to treatment and changes in condition to relevant personnel.
- Offer ongoing education and supervision to clients, families, and other healthcare team members.
- Monitor and document client care services via electronic charting.
- Participate in education programs, quality assessment activities, and staff meetings as required.
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
- Strong problem-solving skills and ability to multitask.
- Excellent communication and interpersonal skills.
- Positive and professional attitude.
