About the RoleWe are looking for a Physical Therapist who is passionate about delivering patient-centered care and making a positive impact on the lives of our patients. As a Home Health Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Conduct thorough assessments of patients' physical abilities and develop personalized care plans to improve their mobility, strength, and overall well-being.
- Perform diagnostic tests and measurements to identify areas of improvement and track patient progress.
- Develop and implement manual therapeutic exercises, gait training, balance, and other interventions to help patients achieve their goals.
- Continuously assess and revise care plans to ensure patients are receiving the best possible care.
- Collaborate with our nursing team to develop and implement multidisciplinary care plans.
To be successful in this role, you will need:
- A current and active license to practice as a Physical Therapist in the state of PA.
- Ability to travel to patient homes as assigned.
