About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Admissions team at Heartland Hospice, a member of the Gentiva family. As a Registered Nurse Admissions, you will play a vital role in explaining our services to patients and families, assessing their needs, and admitting them to our care.

Responsibilities
  • Utilize the nursing process to contribute to each patient's total plan of care.
  • Visit patients and families in their place of residence or in the hospital to assess for appropriateness, explain services, monitor condition, or admit to hospice services in accordance with the admission procedure.
  • Participate in evening/weekend on-call as required.
Requirements

To be successful in this role, you will need:

  • A Registered Nurse license in the state where the program is located.
  • A minimum of 1 year general nursing experience, with Hospice experience preferred.
  • CPR Certification.
