Job Summary

The Occupational Therapist contracted through Symbii Home Health and Hospice is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.

Key Responsibilities
  1. Assume responsibility for ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients, representatives, and caregivers.
  2. Provide services ordered by physicians as indicated in the plan of care.
  3. Offer patient, caregiver, and family counseling.
  4. Provide patient and caregiver education.
  5. Communicate with physicians responsible for home health plans of care and other healthcare practitioners as necessary.
  6. Evaluate patients' functional status and home environments for hazards or barriers to independent living.
  7. Identify equipment needs and participate in the development of total plans of care.
  8. Develop treatment programs and establish goals for improved function.
  9. Teach new skills or retrain patients in daily activities lost due to illness or injury.
  10. Fabricate splints and instruct patients in the use of adaptive equipment.
  11. Train patients in the use of prosthetic and/or orthotic devices.
  12. Maintain clinical records, notes, and reports to physicians on patient condition changes.
  13. Participate in HHA's quality assessment and performance improvement program and in-service training, case conferences, and meetings as required.
  14. Identify patient and family/caregiver needs for other home health services and refer as necessary.
  15. Instruct patients' families/caregivers and other Organization health care personnel in treatment regimes when indicated.
  16. Supervise Certified Occupational Therapy Assistants according to organization policy and state regulations.
  17. When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
  18. Participate in discharge planning for patients.
Job Requirements
  1. Must be licensed where the HHA is located if licensure is required.
  2. Must have graduated from an OT program accredited by the Accreditation Council for Occupational Therapy Education.
  3. Must be eligible for or have taken the OT certification examination administered by the National Board for Certification in Occupational Therapy.
  4. Two years of clinical experience as an occupational therapist.
  5. Demonstrates good verbal and written communication, and organization skills.
  6. Possesses and maintains current CPR Certification.
  7. The ability to drive and/or have access to transportation for agency-related travel is an essential function of the job.
