Job Summary

Under the supervision of the Perinatal Program Manager, the Perinatal Health Worker will provide care coordination and case management for assigned perinatal patients. This includes assessments, information and referral, follow-up, and other supportive services.

Key Responsibilities
  • Provide case coordination and case management for assigned patients
  • Serve as a guide for each assigned patient through the perinatal program
  • Meet each assigned patient at least once each trimester and at least once after delivery and at other necessary appointments as needed
  • Complete health education, nutrition, and psychosocial assessments for initial, trimester reassessments, and postpartum assessments
  • Provide information, referral, and follow-up for patients
  • Provide multilingual perinatal education workshops and other supportive services as appropriate
Requirements
  • A high school diploma with two years' related experience or an equivalent combination of education and experience
  • Must be at least 18 years of age and be willing to undergo training to understand the perinatal program and provide assessment and referral services
  • Bilingual ability in Cantonese or Vietnamese
Preferred Qualifications
  • GED required with college courses in health-related science preferred
  • Commitment to working in the Asian community and community health care with familiarity with community health clinics preferred
