Licensed Vocational Nurse

As a Licensed Vocational Nurse (LVN) with Choice Home Health, you will play a vital role in providing high-quality patient-centered care in the comfort of patients' homes. We are seeking a skilled and compassionate LVN to join our team and deliver exceptional care to our patients.
Responsibilities:
• Provide direct patient care visits, observing, performing, and reporting on preventative, treatment, and rehabilitative nursing procedures while implementing the plan of care.
• Observe and report on the home environment, making recommendations to enhance patient care, promote safety, and prevent adverse effects.
• Work with patients, families, and healthcare team members to coordinate services, promote quality of care, and positive patient outcomes.
• Take verbal orders from healthcare providers as necessary and within the LVN scope of practice in a timely manner and document accordingly.
• Inform healthcare providers of changes in patients' conditions in accordance with providers' orders and document communications accordingly.
• Provide care as ordered, including preventive, rehabilitative, therapeutic, comfort, and pain interventions.
• Provide instructions to patients, caregivers, and staff in a timely manner.
Qualifications:
• Graduation from an accredited nursing program and current state license as a Licensed Vocational/Practical Nurse.
• At least one year of clinical practical nursing experience; home health experience preferred.
• Must have a vehicle, valid current driver's license, and automobile insurance.
• Ability to make home visits and provide skilled nursing assessments and care in the home.
