ChristianaCare is seeking a skilled Registered Nurse to join our Hospital Care at Home Program. As a key member of our team, you will provide high-quality patient care and support to patients in the comfort of their own homes.
Key Responsibilities
- Provide clinical monitoring and care for patients, leveraging technology tools and software to ensure seamless care delivery.
- Assess patient needs, develop care plans, and collaborate with healthcare providers to ensure optimal patient outcomes.
- Manage clinical protocols, escalate concerns to providers as needed, and recommend services for patients.

Requirements
- RN License in Delaware or Compact State.
- BSN preferred or committed to acquiring BSN within 3 years of hire date.
- At least two years of med/surg experience as an RN required.
- BLS required.
Physical Demands
Frequent exposure to biological elements with occasional exposure to chemical and environmental elements.
