Job Title: Human Services Case Manager

Job Summary:

The Oregon Department of Human Services is seeking a dedicated and compassionate individual for a Human Services Case Manager position. As a key member of our team, you will play a vital role in determining initial and ongoing Medicaid eligibility for long-term services and supports, developing and implementing service plans, and coordinating the provision of services for community-based care and nursing homes.

Key Responsibilities:

  • Conduct on-site field reviews using OR ACCESS, CA/PS assessment tools, and laptops to assess client needs and develop individualized case management plans.
  • Provide ongoing case management by coordinating the provision of services to clients according to rules, regulations, and individual case plans.
  • Conduct direct and indirect contacts with consumers or their authorized representatives to ensure effective communication and service delivery.
  • Use OR ACCESS computer systems to input client information, document all aspects of the case, and verify, collect, and enter accurate data for benefit issuance.

Requirements:

  • A bachelor's degree in a Behavioral Science, Social Science, or a closely related field, or a bachelor's degree in any field with one year of human services-related experience.
  • One year of human services-related experience, or an associate degree in a Behavioral Science, Social Science, or a closely related field with two years of human services-related experience.
  • Three years of human services-related experience.

Working Conditions:

  • The work schedule for this position is a 40-hour work week, with possible overtime work before and after scheduled hours and on weekends.
  • The position requires a valid driver's license and an acceptable driving record, as well as the ability to travel to homes of clients, hospitals, nursing facilities, and other field offices.
