Job Summary

We are seeking a highly skilled and compassionate Patient Services Coordinator to join our team. As a Patient Services Coordinator, you will play a critical role in ensuring the timely and efficient delivery of patient care services.

Key Responsibilities
  • Coordinate patient assessment visits, including Oasis visits, to ensure timely completion.
  • Assign and schedule clinicians in the most efficient geography, maximizing clinician efficiency and optimizing capacity.
  • Collaborate with field clinicians to ensure all patient visits are scheduled and completed as ordered and within the care center.
  • Manage calls from patients and field staff related to scheduling issues.
  • Refer clinical and performance-related issues to the clinical manager.
  • Monitor hospitalized patients and communicate with the team to ensure timely follow-up.
  • Support the clinical manager and collaborate with patients, clinicians, referral sources, business development team, and other office personnel to ensure timely communication.
  • Run applicable reports and respond to workflow, taking appropriate actions.
  • Assist with internal or external transfer of patients between care centers and/or hospice services.
  • May be responsible for maintaining supply closets with routine supplies, ensuring supplies are within expiration dates and packaged appropriately, and serving as liaison with the field for patient supply needs.
Requirements
  • High School diploma or GED equivalent.
Preferred Qualifications
  • One year of scheduling experience in a healthcare environment.
  • One year of administrative experience in a healthcare environment.
