Job Title: Medical Social Worker Hospice Care Specialist

We are seeking a compassionate and skilled Medical Social Worker to join our Hospice team. As a Medical Social Worker, you will play a vital role in providing emotional and social support to patients and their families, helping them navigate the complexities of end-of-life care.

About the Role:
  • Provide medical social services under the direction of a physician and Interdisciplinary Group to assist in understanding significant social and emotional factors related to the patient's health status.
  • Assist patients, significant others, physicians, and healthcare team staff to understand personal, emotional, environmental, and social factors related to the patient's health status.
  • Contribute to the development of a comprehensive, integrated Plan of Care for patients daily.
  • Instruct healthcare team members on community resources available to assist patients.
  • Establish a Plan of care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery.
Requirements:
  • Master's Degree from a school of Social Work accredited by the Council of Social Work Education.
  • One year of social work experience in a healthcare setting.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • OR: Must be licensed as a Social Worker from the State Board of Social Work.
