The Physician Assistant (PA) or Nurse Practitioner (NP) will demonstrate a strong commitment to delivering exceptional primary and preventive healthcare. This role emphasizes continuity of care, fostering enduring relationships with patients, and enhancing health promotion initiatives while managing intricate chronic health issues. Key responsibilities include:
- Delivering comprehensive outpatient primary care services for both adults and children.
- Conducting thorough assessments to establish accurate diagnoses based on patient history and physical examinations.
- Initiating, evaluating, and adjusting treatment plans essential for patient health.
- Collaborating effectively with a multidisciplinary team, including one supervising physician and four health coaches dedicated to patient wellness.
- Coordinating and expanding a network of reliable external medical professionals, including hospitalists and emergency care providers.
- Leading or participating in community-based initiatives aimed at enhancing patient engagement, access to care, and clinical quality metrics.
- Consulting with the supervising physician on cases as outlined in the delegation agreement.
- Travel to various clinics and wellness centers is a requirement.
- Performing additional responsibilities as assigned.
- Possession of a valid California Physician Assistant (PA) license or California Nurse Practitioner (NP) license.
- Current DEA registration is mandatory.
- Proficiency in technology, including MS Office, tablets, smartphones, and electronic medical record systems.
- Fluency in Spanish is highly preferred, with a requirement for Medical Spanish proficiency.
